The items listed below MUST be completed and mailed to the LSBME as part of the reinstatement application process.
The Board may issue a license which has expired to a physician for reinstatement provided that application for reinstatement is made within four years of the date of expiration
**If it has been 10 years since your have taken a certification examination and you are not currently Board certified or currently licensed in another state, you may be required to take the SPEX examination.
Oath or Affirmation
All yes answers must be accompanied by a sworn affidavit. A sworn affidavit is an explanation (in applicant’s own words) that must be typed and notarized. Note: If the Oath or Affirmation is incomplete or inaccurate, or if criminal history is found that you did not disclose, you will be required to submit a new Oath or Affirmation, a notarized affidavit as to why you did not disclose the information and a new processing fee equal to the initial licensure fee. It is important that you answer all questions fully, accurately and truthfully. Any discrepancy, inaccuracy or incomplete information on the Oath or Affirmation may be viewed by the Board as attempting to obtain a license by means of fraud, and may be a reason for the license application to be denied. Do not take the advice of friends, lawyer, etc.
Additional Requirements
Fees - Fee must be paid via credit/debit card (Visa, MasterCard or Discover only). Once your application is processed, your analyst will invoice you for the balance. Fee is non-refundable.
0-2 years- $632.00
2-3 years $932.00
3-4 years $1,232.00
Continuing Education - Submit 20 hours of Category 1 AMA Board approved CME's for each year since the date the license was last renewed.
Practice History - List the practice history and non-medical/professional related activities since your license expired. Do not include training.
Verification of Licenses in Other States
Verification of state licenses are required. Contact the state board(s) directly to obtain verification of your license. Please note, there may be a processing fee needed.
Verification of ABMS or AOA Board Certification/ Recertification - Verification of ABMS or AOA Board Certification/ Recertification may be required if you are not currently licensed in another state or have taken certification examinations over ten years ago. If you are not currently Board certified and do not currently hold an active license in another state, you may be required to take the SPEX.
